Cloxacillin is indicated for the treatment of infections caused by gram-positive bacteria and penicillinase-producing Staphylococci.
250 mg to 500 mg every 6 hours, depending on the nature and severity of the infection. In more serious infections, the dose may be increased to 1 g three times daily.
50 to 100 mg/kg per day, divided into doses given every 6 hours, up to a maximum of 4 g per day. For children aged 10 years and above, 15 mg/kg three times daily (maximum 3 g daily) is a commonly used regimen.
The safety and efficacy of Cloxacillin in children under 1 year of age have not been established.
All doses should be taken 1 to 2 hours before meals or on an empty stomach to ensure adequate absorption.
Reconstitute the powder with the specified volume of water as indicated on the label or by the pharmacist, and shake well before use.
Administer the suspension between meals — at least 1 to 2 hours before eating or on an empty stomach — to ensure adequate absorption, as food reduces bioavailability.
Measure each dose carefully using the supplied measuring device.
Cloxacillin belongs to the isoxazolyl penicillin group of beta-lactam antibiotics. It is specifically engineered to resist beta-lactamase (penicillinase), making it effective against penicillinase-producing organisms, particularly Staphylococcus aureus.
Cloxacillin is rapidly but incompletely absorbed from the gastrointestinal tract. Absorption is reduced in the presence of food, so the drug should be taken between meals.
Gastrointestinal: Nausea, vomiting, diarrhoea. Rarely, pseudomembranous colitis has been reported.
Allergic reactions: Hypersensitivity reactions ranging from mild rash to severe anaphylaxis.
Haematological: Neutropenia, leukopenia, and thrombocytopenia. Neutropenia is more common with higher doses and longer durations of therapy (most often after 14 days), and is reversible upon discontinuation.
Hepatic: Transient increases in serum transaminases and, rarely, cholestatic hepatitis. Liver enzymes may take several weeks to return to normal after stopping the drug.
The safety and efficacy of Cloxacillin have not been established in infants below 1 year of age.
Elderly patients, particularly those with reduced renal function, are at higher risk of drug accumulation and associated toxicity, including neurological effects. Dose monitoring is advisable.
No routine dose adjustment is required for mild to moderate renal impairment; however, patients with creatinine clearance below 10 mL/min (with or without haemodialysis) are at significantly increased risk of overdose-related toxicity. Therapeutic drug monitoring and dose reduction should be considered in these patients.
Pregnancy: Cloxacillin is classified as Pregnancy Category B. No controlled studies have been conducted in pregnant women, but no reports of congenital abnormalities associated with its use have appeared in the literature. It should be used during pregnancy only when clearly indicated.
Lactation: There are no specific data on the excretion of Cloxacillin into human breast milk. As with other penicillins, excretion in small amounts is possible, but adverse effects in nursing infants are considered unlikely. Use with caution in breastfeeding women.
There is no specific antidote for Cloxacillin overdose. Management is supportive: discontinue the drug and promote elimination. Haemodialysis may assist in removing the drug in cases of serious toxicity, particularly in patients with renal impairment.
High serum concentrations, especially in patients with renal failure, may cause neurological effects including delirium and persistent coma. Patients with severely reduced renal function receiving high doses should be closely monitored.
Store in a cool, dry place at 15–30°C, protected from light.
After reconstitution, the suspension should be stored as directed on the label (typically refrigerated) and discarded after the period stated by the manufacturer or pharmacist.
Keep out of reach of children.
